Dr. Lanzer wants to reduce the risk of identity theft in her practice. She has decided to stop collecting and retaining Social Security and Driver's License numbers on the client information sheets. What should Dr. Lanzer do with the Social Security and Driver's License numbers that were previously collected and now reside on the client information sheets in her medical records?

WHAT SHOULD SHE DO?

    Solution 1:
      Develop a new Client Information Sheet that does not contain Social Security and Driver’s License numbers. Pull all previous medical records and transfer the client information, minus the Social Security and Driver's License numbers, to the new Client Information Sheet. Cross-cut shred the old Client Information Sheet that contains the Client’s Social Security and Driver’s License numbers.

    Solution 2:
      Develop a new Client Information Sheet that does not contain Social Security and Driver’s License numbers and leave the previously collected private personal information alone.

    Solution 3:
      Instead of collecting the Social Security and Driver’s License numbers, ask clients to provide their Checking Account number on the Client Information Sheet.
